we wish to inform you that the "European Regulation 2016/679 on the protection of individuals with regard to the processing of Personal Data and on the free movement of such data" (hereinafter "GDPR") provides for the protection of persons and other subjects with regard to the processing of personal data. Adeodata SRL and Adeodata SA as joint data controllers, in accordance with Article 13 of the GDPR, therefore, provide you with the following information:

  1. CATEGORIES OF DATA: Adeodata SRL and Adeodata SA as joint data controllers will process the personal and/or sensitive and health-related data that will be entered in the form.
  2. SOURCE OF PERSONAL DATA: The personal data that Adeodata SRL and Adeodata SA will hold is collected directly from the person concerned when filling in this form.
  3. DATA CONTROLLER: The data controllers are: Adeodata SRL, VIA Carducci, 2 - 20123 MILAN, VAT no. 05617310965 contactable by phone on +39 02 96720933 or by email, and Adeodata SA, Via Calgari, 2 - CH 6900 LUGANO IDE-CHE 112203579 contactable by phone on +41 91 9803318 or by email represented in Europe, as non-European Data Controller, by Adeodata SRL.
  4. D.P.O. CONTACT DETAILS: The Data Controllers are not required to appoint a D.P.O.
  5. PURPOSE OF DATA PROCESSING AND LEGAL BASIS: The processing of your data, collected and archived in relation to the completion of this form, has as its legal basis your consent and is carried out for the following purposes:requesting information.
  6. RECIPIENTS OF THE DATA: Within the limits pertinent to the processing purposes indicated, your data may be communicated to partners, consulting companies, private companies, appointed as Data Processors. Your data will not be disseminated in any way. The Data Processors and Persons in Charge of Processing in office are punctually identified in the Privacy Document, which is updated periodically.
  7. TRANSFER OF DATA ABROAD: The data collected may be communicated to the foreign associates of the Data Controllers or to Data Processors located in third countries, or to other companies in third countries, by virtue of an adequacy decision of the Commission or in the presence of appropriate guarantees.
  8. PERIOD OF STORAGE: The data collected will be retained for a period of time not exceeding the achievement of the purposes for which they are processed ("principle of limitation of storage", art.5, GDPR) or in accordance with the deadlines provided for by law. A check on the obsolescence of the retained data in relation to the purposes for which they were collected is carried out periodically.
  9. RIGHTS OF THE DATA SUBJECT: The data subject shall always have the right to request from the Data Controllers access to his/her data, rectification or erasure of data, restriction of processing or the possibility to object to processing, to request data portability, to withdraw consent to processing by asserting these and the other rights provided by the GDPR by simple communication to the Data Controllers. The data subject may also lodge a complaint with a supervisory authority.
  10. OBLIGATORY OR NON-ATTRIBUTION OF CONSENT: We inform you that the provision of data is optional for some fields and mandatory for others (marked with an asterisk), and failure to provide such data will result in the form not being sent.
  11. DATA PROCESSING METHODS: The personal data that you provide will be processed in compliance with the above-mentioned regulations and with the confidentiality obligations that inspire the activity of the Joint Data Controllers. The data will be processed both by computer and on paper or any other suitable support, in compliance with adequate technical and organisational security measures provided for by the GDPR.
